New Patient Registration

To register as a patient you will need to call in during reception opening hours to complete documentation.

We are happy to register you as a patient at the surgery provided that you live within our practice area. If in doubt as to whether you live within our area please contact reception who should be able to help.

You can only be registered as a patient at one GP Surgery so if you register with us you will be de-registered from your current surgery and your previous medical notes will be passed to us.

Please come to reception and ask for an application form. It would assist us if you are able to bring your Medical Card with you or alternatively your NHS number (you can get this from your current GP practice).

When you register we will arrange an appointment for a health check.  This is important as it allows us to get to know you better and collect important information on past medical health, current problems and medication and is an excellent time to ensure all immunisations, contraceptive requirements, cervical smears and blood pressure measurements are up to date. As there can be a delay in obtaining your records from your previous doctor this information is vital to your new doctor.
